Job summary

Logan Square Rehabilitation and Healthcare Center is seeking an experienced MDS Coordinator to oversee accurate MDS assessments and ensure regulatory compliance. The role requires a registered nurse with at least three years in long-term care and prior MDS/RAI experience.

The coordinator will supervise data entry, validate submissions, generate reports for leadership, and collaborate with interdisciplinary teams to optimize resident care and reimbursement outcomes.

Qualifications

• RN license and a nursing degree required.
• Minimum of three years of clinical experience in a long-term care setting.
• Prior MDS/RAI experience required.
• Strong clinical assessment skills with attention to detail and accuracy.
• Working knowledge of Medicare and Medicaid regulations and reimbursement systems.

Responsibilities

• Ensure timely and accurate completion of all MDS assessments per regulations and deadlines.
• Supervise MDS data entry, validation, and transmission for accuracy and timeliness.
• Identify and resolve data discrepancies and validation issues.
• Prepare MDS-related reports for the Director of Nursing and leadership.
• Collaborate with clinical teams to ensure accurate resident care representation.
• Support audits and regulatory reviews; assist with responses to surveys.
