Responsibilities

• Collaborate with the interdisciplinary care team to ensure timely completion of MDS assessments and care plans.
• Conduct resident interviews and gather information to inform MDS assessments and care plans.
• Maintain accurate and up-to-date records of resident assessments and care plans.
• Provide education and support to the care team on MDS-related documentation and assessment processes.
• Participate in quality improvement initiatives to optimize MDS processes.
