Job summary

Qualifications

• MDS and care-planning experience in a long-term care setting
• Strong organizational, communication, and documentation skills
• Ability to coordinate effectively across clinical disciplines and meet deadlines

Responsibilities

• Coordinate and complete resident assessments in accordance with applicable requirements and established timelines
• Gather and review clinical information to ensure MDS documentation is accurate and complete
• Collaborate with nursing staff and the interdisciplinary team to develop and update resident care plans
• Monitor assessment schedules and help ensure documentation and submissions are completed on time
• Communicate assessment findings and related care needs with appropriate team members
• Provide guidance to staff on MDS processes and documentation
• Maintain organized, accurate records and follow privacy and compliance standards
