Job summary

Northern Manhattan Rehabilitation & Nursing Center is hiring an MDS Coordinator (RN) to coordinate and complete MDS assessments for accurate care planning and regulatory compliance. You will support high-quality, resident-centered care and work with an interdisciplinary team to ensure proper documentation and timely submissions.

The role requires a valid NY RN license, knowledge of the RAI process, and strong organizational and communication skills.

Qualifications

• Experience coordinating and completing MDS assessments
• Strong knowledge of RAI processes, care planning, and Medicare/Medicaid guidelines
• Excellent organizational, analytical, and documentation skills
• Strong communication and teamwork abilities
• Regulatory compliance focus in a skilled nursing setting

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state regulations
• Develop and update comprehensive care plans with the interdisciplinary team
• Ensure accuracy and timeliness of all MDS documentation and submissions
• Monitor clinical documentation to support appropriate case mix and reimbursement
• Participate in care plan meetings and communicate resident needs effectively
• Collaborate with nursing, therapy, and administrative staff to ensure regulatory compliance
• Current RN license in NY and prior MDS experience preferred
