Job summary

Life Care Centers of America is seeking an RN MDS Coordinator to oversee the coordination and submission of MDS Assessments, CCAs, and Care Plans in line with CMS regulations and Life Care standards. You will work within an interdisciplinary team, ensuring accurate documentation and timely reporting of changes in patient status.

Qualifications

• Two (2) nursing experience. Geriatric nursing experience preferred.
• CPR certification upon hire or during orientation; CPR must remain current.

Responsibilities

• Coordinate and assist with completion and submission of interdisciplinary MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations.
• Report changes in a patient’s condition identified by the MDS Assessment to the DON.
• Provide education to direct care associates regarding CMS RAI Manual updates that impact documentation.
• Review Interdisciplinary Comprehensive Care Plan and Final Validation Reports for CMS QIES submission before billing.
• Attend PPS/Triple Check and other meetings; participate as needed.
• Perform staff nurse functions as required; exhibit excellent customer service and professionalism.
