Job summary

Avamere Rehab of Cascade Park in Vancouver, WA seeks an MDS Coordinator - Registered Nurse to lead review and coordination of resident care plans, ensuring quality care and effective communication with the clinical team.

The role requires strong knowledge of the MDS/RAI process, nursing licensure, and experience with electronic medical records. This full-time position offers daytime Mon-Fri scheduling and a competitive salary range based on experience.

Qualifications

• Nursing degree and active RN license in state.
• Knowledge of MDS/RAI processes and regulatory requirements.
• Experience with electronic medical records and documentation.
• Ability to coordinate care plans and communicate with clinical teams.

Responsibilities

• Review resident care plans for goals, problems and revisions.
• Ensure care plans are communicated to all care staff.
• Schedule and facilitate care plan conferences.
• Monitor PPS and OBRA assessments and ensure accurate documentation.
• Ensure compliance with state and federal guidelines, including MDS processes.
• Oversee the MARs, flow sheets, and physician orders.
• Coordinate with other departments to ensure consistent resident care.
