Job summary

Cascadia Healthcare seeks an experienced MDS Coordinator (RN) to ensure timely and accurate assessments and care plans in a long-term care setting at the Clarkston, WA location.

You will lead interdisciplinary teams, coordinate MDS development and PPS planning, and ensure regulatory compliance while communicating with families and staff. RN licensure and CPR certification are required.

Qualifications

• Knowledge of CMS guidelines for MDS.
• Demonstrated ability to implement the Nursing department's programs and procedures.
• Knowledge of nursing practices and the long-term care regulations.

Responsibilities

• Manages MDS assessments based on CMS guidelines.
• Coordinates, records, updates, and monitors all MDS.
• Participates in clinical meetings.
• Coordinates in patient/family care conferences for residents.
• Collects and reports data on resident encounters and inter-institutional issues.
• Coordinates communication between residents, family, medical staff, admin, and payor source.
• Develops written plans of care with measurable objectives for each resident.
• Understands MDS/PPS rules and Utilization Guidelines for each assessment.
• Ensures MDS and RUG accuracy, encoding, editing, printing, and export per state/federal rules.
• Assists with staff education for documentation quality.
• Reviews quality metrics with clinical leadership for QA action planning.
• Monitors quality reporting data to ensure accuracy and avoid penalties.
• Maintains punctuality and attendance for shifts.
