Job summary

The Creston Health & Rehabilitation in the Portland area is seeking an on-site MDS Coordinator to manage the RAI/MDS process, coordinate interdisciplinary care planning, and ensure accurate documentation for compliant reimbursement and quality measures.

As a full-time, on-site role, you will review records, update care plans, and educate staff on MDS-related procedures while supporting audits and quality initiatives in a collaborative team environment.

Qualifications

• Active Licensed Practical Nurse (LPN) license in good standing.
• Competence in MDS and RAI processes including completion, coding and regulatory compliance.
• Proficiency in care planning and updating individualized care plans.
• Experience or interest in geriatric nursing and long-term care regulations.
• Ability to interpret clinical documentation, analyze data, and maintain EHR.
• Strong communication and collaboration with interdisciplinary teams.
• Strong organizational skills and ability to manage multiple assessments and deadlines.
• Prior long-term care experience preferred; Medicare/Medicaid reimbursement knowledge is a plus.

Responsibilities

• Review clinical records and identify changes in resident status.

• Enter data into the electronic health record system.
