Job summary

Avamere Rehab of Cascade Park in Vancouver, WA seeks a full-time MDS Coordinator - Registered Nurse to lead care plan reviews and coordinate patient assessments in compliance with state and federal regulations. The role requires an active RN license (WA preferred; OR also), MDS/RAI knowledge, and EMR experience.

You will work with the nursing team to ensure accurate documentation, PPS/OBRA assessments, and a seamless care-planning process.

Qualifications

• Nursing degree from an accredited college or university.
• Maintain a current, active RN license in the relevant state.
• Oregon: RN is required.
• Washington: RN is preferred.
• MDS certification preferred.
• Knowledge of RAI, CAAs, RUG categories, and MDS 3.0.
• Knowledge of nursing practices, laws and guidelines for nursing care communities.
• Experience with electronic medical records and computer documentation systems.

Responsibilities

• Review resident care plans for goals, problems, approaches, and revisions.
• Ensure all personnel involved in care are aware of care plans.
• Schedule and facilitate care plan conferences.
• Monitor and guide PPS and OBRA assessments.
• Ensure accuracy of clinical assessments and documentation.
• Ensure compliance with state and federal guidelines including MDS processes.
• Manage the UR (utilization review) process.
• Oversee MARs, treatment, flow sheets and physician orders.
• Document nurses' notes reflecting care provided and resident response.
• Coordinate with other departments to ensure consistency of resident care.
• Participate in facility surveys by authorized government agencies.
• Report findings and make recommendations to management.
• Support Administrator and Director of Nursing.
