Genesiscareers in Skowhegan, Maine, is seeking a Registered Nurse to join our long-term care team. You will participate in Patient Assessment Process, help develop individualized care plans, and ensure accurate MDS completion and CAAs.

Qualifications

• Must be a Registered Nurse currently licensed by the state.
• Previous experience in a hospital, long-term facility, or other healthcare facility is required.
• Prior experience completing MDSs and Care Plans is required.
• Knowledge of general, rehabilitative and restorative nursing and related regulations governing long-term care.

Responsibilities

• Participate in the Patient Assessment Process through completion of items on the MDS and CAAs.
• Assist in developing individualized patient care plans and identify patient needs and outcomes.
• Contribute to the RAI process with adherence to guidelines for completion, encoding, and transmission of the MDS.
• Identify changes in patient condition with the interdisciplinary team and coordinate assessments.
• Assist with Activities of Daily Living training for staff and ongoing education.
• Attend educational programs to stay current with RAI/Prospective Payment System regulations.
