Job summary

Maine Veterans’ Homes is seeking an experienced Registered Nurse (RN) to join our team as a Per Diem MDS Coordinator. This is an on-site role focusing on accurate MDS assessments and collaboration with the interdisciplinary team to ensure residents receive the care and services they need.

You will coordinate the MDS process, review documentation, and perform audits while working independently. We require current Maine RN license, CPR, and long-term care experience; prior MDS or geriatric nursing

Qualifications

• Current, unrestricted RN license in Maine.
• CPR certification.
• Experience in a long-term care or skilled nursing environment.
• Proficiency with MS Word and Excel.
• MDS coordination or geriatric nursing experience preferred.

Responsibilities

• Coordinate and complete the MDS assessment process.
• Collect, reconcile, and validate clinical information for MDS.
• Conduct MDS audits for accuracy, completeness and compliance.
• Collaborate with nursing leadership and interdisciplinary teams.
• Maintain records and documentation per MVH policies and regulatory requirements.
• Work independently while serving as a knowledgeable resource to teams.
