Glenburnie Health & Rehab Center in Richmond, VA is seeking an RN MDS Coordinator to ensure accurate MDS assessments and to work with the interdisciplinary team to create comprehensive care plans after reviewing medical records and coordinating with staff, physicians, and families.

The role emphasizes compliance with ARD timelines and Medicare/Medicaid guidelines, collaboration with nursing leadership, and active participation in quality measure initiatives.

Qualifications

• Active RN licensure required.
• MDS experience in skilled nursing/long-term care preferred.
• Proficient with Microsoft Office Suite.
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Complete and submit accurate minimum data set (MDS) assessments on patients.
• Oversee care plan process including coordination of patient care plan conferences.
• Ensure ARD stays within Medicare/Medicaid guidelines.
• Participate in monthly Quality Measure meetings.
• Work with nursing leadership to identify and resolve documentation opportunities.
• Assist interdisciplinary team in Medicare coverage decisions.
