The role emphasizes adherence to Medicare/Medicaid guidelines, participation in quality measure meetings, and supporting documentation improvements while maintaining strong communication with staff, physicians, and families.

Qualifications

• Active Licensure as a Registered Nurse (RN).
• Skilled Nursing / Long-Term Care MDS experience preferred.
• Proficient with Microsoft Office Suite (Word, Excel, etc.).
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Complete and submit accurate minimum data set (MDS) assessments on patients.
• Oversee care plan process including coordination of patient care plan conferences.
• Ensure ARD stays within Medicare & Medicaid guidelines.
• Participate in monthly Quality Measure meetings.
• Work with nursing leadership team to identify and resolve documentation opportunities.
• Assist interdisciplinary team in making appropriate Medicare coverage decisions.
