Beaufont Health & Rehabilitation Center in Richmond is seeking a full-time MDS Coordinator with RN license for our 120-bed skilled nursing facility. The role centers on completing MDS assessments, creating comprehensive care plans, and collaborating with care teams to ensure regulatory compliance.

The ideal candidate will have strong clinical assessment skills, the ability to make independent decisions, and excellent communication with staff, physicians, and families.

Qualifications

• RN license and skilled nursing/rehabilitation experience.
• MDS 3.0 process knowledge.
• Strong communication and customer service skills.
• Ability to work with an interdisciplinary team.

Responsibilities

• Complete minimum data set (MDS) assessments and develop plans of care.
• Review patient records and coordinate with staff, physicians, and families.
• Ensure compliance with federal and state regulations and company policies.
• Maintain accurate documentation and participate in quality improvement initiatives.
