Westerly Rehabilitation and Healthcare Center is seeking an experienced MDS Coordinator. This full-time role requires accurate MDS/RAI assessments to support care planning, reimbursement, and regulatory compliance, with leadership over data entry and reporting to the DON and leadership team.

Qualifications
