Job summary

Twin Oaks Rehab is seeking an RN MDS Coordinator in Massachusetts to lead MDS assessments and interdisciplinary care planning. You will ensure compliance with federal/state regulations and mentor nursing staff on MDS processes.

The role emphasizes accuracy, collaboration with the care team, and liaison with billing to support compliant claims. Monday–Friday schedule and growth opportunities are highlighted.

Qualifications

Current RN license in MA required.
Experience as MDS Coordinator in skilled nursing preferred.
Strong understanding of RAI, PDPM and Medicare/Medicaid regulations.

Responsibilities

Complete and submit MDS assessments accurately and on schedule.
Coordinate and lead interdisciplinary care plan meetings.
Ensure compliance with federal and state MDS regulations.
Review medical records with nursing and other staff for accuracy.
Monitor RAI process and update assessments as needed.
Participate in quality assurance initiatives.
Collaborate with billing/reimbursement teams to support accurate claims.
Serve as clinical resource and mentor to nursing staff on MDS processes.
