Job summary

Twin Oaks Rehab and Nursing in Massachusetts is seeking an experienced RN MDS Coordinator to oversee MDS submissions, lead care planning, and ensure regulatory compliance. This role emphasizes clinical accuracy, leadership, and collaboration with nursing staff to support residents’ needs.

You will complete MDS assessments, coordinate interdisciplinary care plan meetings, and ensure compliance with federal/state MDS regulations while collaborating with billing to support accurate claims.

Qualifications

• Current RN license in Massachusetts (Required)
• Prior experience as an MDS Coordinator in a skilled nursing facility (Preferred)
• Strong understanding of RAI process, PDPM, and Medicare/Medicaid regulations
• Excellent organizational and communication skills
• Ability to work collaboratively in a team setting

Responsibilities

• Complete and submit MDS assessments accurately and on schedule
• Coordinate and lead interdisciplinary care plan meetings
• Ensure compliance with all federal and state MDS regulations
• Review medical records and consult with nursing and other RN and non RN staff to ensure accuracy
• Monitor RAI process and update assessments as needed
• Participate in quality assurance initiatives
• Work closely with billing and reimbursement teams to support accurate claims
• Serve as a clinical resource and mentor to nursing staff related to MDS processes
