Cura of Le Sueur is seeking an experienced MDS Coordinator to join our nursing leadership team. You will oversee the MDS/RAI process, ensure accurate assessments and care plans, and collaborate with an interdisciplinary team to optimize resident outcomes and regulatory compliance.

Ideal candidates bring a Minnesota RN license, strong MDS experience, and knowledge of PDPM and reimbursement. This role emphasizes detail, coordination, and staff education within a skilled nursing facility.

Qualifications

Must hold an active Minnesota RN license.
Strong MDS/RAI experience with focus on accurate documentation and regulatory compliance.
Knowledge of MDS 3.0, RAI, PDPM, and Medicare/Medicaid reimbursement.
Excellent attention to detail and ability to coordinate interdisciplinary collaboration.
Ability to educate nursing staff on documentation requirements and processes.
Must meet background check, health, and employment requirements.

Responsibilities

Coordinate and complete MDS assessments and RAI processes in accordance with federal/state requirements and timelines.
Maintain accurate MDS schedules and ensure timely submissions.
Collaborate with nursing, therapy, social services, dietary, and other teams for comprehensive care planning.
Review clinical documentation for accuracy of resident conditions, services, and functional status.
Identify documentation gaps and drive quality improvements in documentation and coding.
Monitor Medicare and skilled nursing documentation to support medical necessity and reimbursement.
Educate staff on MDS-related processes and HIPAA compliance.
Participate in interdisciplinary meetings and quality improvement initiatives.
Assist with audits, surveys, and requests for supporting clinical information.
Maintain confidentiality and adherence to facility policies.
