Job summary

Cura of Le Sueur is seeking an experienced MDS Coordinator to join our nursing leadership team. You will oversee the MDS/RAI process, ensuring accurate assessments, care planning, and regulatory compliance to support appropriate reimbursement.

Ideal candidates hold an active MN RN license and have experience in skilled nursing. You will collaborate with nursing, therapy, social services, dietary, and other teams to protect resident well-being and drive quality outcomes.

Qualifications

• Current and active RN license in Minnesota.
• Experience in skilled nursing or long-term care preferred.
• Understanding of MDS 3.0, RAI, and CMS requirements preferred.
• Strong organizational and time-management skills.
• Excellent written and verbal communication.

Responsibilities

• Oversee and coordinate the resident assessment process to reflect needs accurately.
• Manage MDS assessments per federal/state requirements and timelines.
• Coordinate with interdisciplinary team for information needed for assessments and care planning.
• Ensure documentation supports medical necessity and appropriate reimbursement.
• Identify gaps and improve documentation accuracy and consistency.
• Support audits, surveys, and reviews with timely, thorough responses.
• Educate staff on documentation requirements and MDS processes.
• Maintain resident confidentiality and HIPAA compliance.
