Job summary

South Mountain Healthcare and Rehabilitation Center is seeking a Full-Time MDS Coordinator to join our dedicated team. The right candidate will work with the Interdisciplinary Team, maintain detailed records, and ensure timely MDS completion using PCC.

Familiarity with PCC, 1-2 years of MDS experience (training available), and a current NJ RN license in good standing are required. Expect collaborative scheduling, regulatory compliance, and ongoing care planning support.

Qualifications

• Current NJ RN License in good standing.
• 1-2 years MDS experience preferred (training available).
• Familiarity with Point Click Care (PCC) software.

Responsibilities

• Schedule and maintain accurate, timely completion of MDS using PCC.
• Ensure all MDS are in compliance with State and Federal requirements.
• Ensure timely and accurate electronic transmission to the state.
• Attend and manage weekly utilization review meetings with therapy and team.
• Participate in developing/updating/reviewing/completion of individualized Care Plans.
• Monitor facility staff regarding the MDS process as needed.
• Input and update appropriate ICD-10 codes for all residents.
• Complete other duties as assigned.
