Job summary

Life Care Centers of America is seeking an experienced RN MDS Coordinator to coordinate and assist with completion and submission of accurate MDS Assessments, CCAs, and Care Plans in line with CMS RAI Manual regulations.

You will ensure timely documentation, educate direct care staff on CMS updates, review interdisciplinary plans, and participate in daily meetings while maintaining professional standards and patient-centered care.

Qualifications

• RN licensure in good standing.
• Associate’s or Bachelor’s in nursing from an accredited program.
• Two years of nursing experience; geriatric experience preferred.

Responsibilities

• Coordinate and assist with completion and submission of MDS Assessments, CCAs, and Care Plans.
• Report changes in a patient’s condition identified by the MDS to the DON.
• Educate direct care staff on CMS RAI updates affecting documentation.
• Review interdisciplinary care plans and ensure CMS reporting accuracy.
• Monitor CMS QIES submissions and notify Business Office when ready to bill.
