Job summary

EPIC Healthcare in the Philadelphia area is seeking an MDS Coordinator to ensure accurate and timely completion of MDS assessments and coordinate resident care planning. You will work with a multidisciplinary team to gather data and develop individualized care plans, while maintaining compliance with state and federal regulations.

The role requires an active RN or LPN license in Pennsylvania, a detail-oriented mindset, and a commitment to quality improvement in a respected nursing home.

Qualifications

• Must be detail-oriented and able to manage complex data.
• Experience coordinating MDS in long-term care preferred.
• Strong collaboration with interdisciplinary team members.

Responsibilities

• Coordinate and oversee MDS assessments for all residents per regulations.
• Collaborate with nurses, therapists and social workers to build care plans.
• Ensure accuracy and timeliness of MDS documentation.
• Review medical records to assess physical, mental and psychosocial status.
• Communicate findings and care plans to families and providers.
• Stay informed on regulatory changes affecting MDS and reimbursement.
