EPIC Healthcare in Trenton, NJ is seeking an experienced MDS Coordinator to oversee the Minimum Data Set process, ensure regulatory compliance, and partner with interdisciplinary teams to support high-quality resident care.

The ideal candidate holds a valid RN or LPN license in New Jersey, has MDS experience, and strong knowledge of PDPM, Medicare/Medicaid, and the RAI process, with excellent attention to detail.

Qualifications

• RN or LPN license in New Jersey
• Prior MDS experience required
• Strong knowledge of PDPM, Medicare/Medicaid, and RAI process
• Excellent attention to detail, communication, and organizational skills
• Experience in post-acute or long-term care settings is a plus

Responsibilities

• Complete and manage the MDS process in compliance with federal and state regulations
• Coordinate care plans in collaboration with interdisciplinary team
• Ensure accurate coding and documentation to support optimal reimbursement
• Monitor CMS quality measures and identify areas for improvement
• Serve as a resource and support for clinical and administrative staff on MDS-related matters
