Job summary

Cavalier Healthcare of Trussville LLC in Alabama seeks an MDS Coordinator to ensure accurate Minimum Data Set assessments for residents, in full compliance with state and federal regulations.

The role involves collaborating with the RN Coordinator, Director of Nursing, and interdisciplinary team to develop individualized care plans, enter data into the system, and support high-quality resident care on the floor.

Qualifications

• Must be an L.P.N. or R.N. currently licensed and in good standing with the Board of Nursing in the state in which the facility is located.

Responsibilities

• Complete the MDS for each resident in accordance with state and federal regulations.
• Enter MDS assessments into the computer system and transmit assessment data.
• Print CAT and CAA reports and complete or assist with their completion.
• Collaborate with the interdisciplinary team to develop and complete individualized resident care plans.
• Enter resident care plans into the computer system and update as needed.
• Communicate with residents, families, physicians, and staff regarding resident status and care planning needs.
• Provide nursing care on the floor when needed.
