Job summary

Cavalier Healthcare of Trussville LLC seeks an experienced MDS RN to lead Minimum Data Set assessments for residents, ensuring accuracy and compliance with state and federal regulations. You will coordinate with physicians and an interdisciplinary team to develop individualized care plans and ensure timely data entry and reporting.

The role requires Alabama RN licensure, strong knowledge of MDS processes, and the ability to work on the floor as needed within a busy nursing facility.

Qualifications

• Must hold an RN license in good standing with the Alabama Board of Nursing.
• Experience with MDS processes and regulatory compliance is preferred.
• Ability to work with interdisciplinary teams to develop individualized care plans.

Responsibilities

• Evaluate resident status to ensure accurate MDS completion.
• Complete MDS timely in accordance with state and federal regulations.
• Enter MDS assessments into the computer system and transmit data per requirements.
• Print CAT and CAA reports and assist with completion.
• Collaborate with the care team to develop and update resident care plans.
• Maintain and update resident care plans in the system.
• Communicate with residents, families, physicians, and staff about care planning.
• Provide nursing care on the floor when needed.
