Job summary

Sans Souci Rehabilitation & Nursing Center in Yonkers, NY is seeking an experienced MDS Coordinator RN to manage MDS, CAAs, and care plans in line with federal and state regulations. You will act as an in-house case manager, coordinating with physicians, families, and third party payers to ensure timely, compliant care planning.

The role requires RN licensure, 2+ years in healthcare, and 1+ year in long-term care, with training available for RN candidates in MDS processes.

Qualifications

• Registered Nurse with current, active license in state of practice.
• Minimum two years of clinical experience in a health care setting.
• Minimum of one year of experience in a long term care setting.
• Prior experience as an MDS coordination accepted.
• Training program available for RN candidates with demonstrated assessment skills.

Responsibilities

• Oversees accurate and thorough completion of the MDS, CAAs and Care Plans in accordance with regulations.
• Acts as in-house Case Manager coordinating services with physicians, families, third party payers and facility staff.
• Communicates regulatory risk and trends to Administrator and Director of Nursing.
• Captures the ARD and determines the RUG score reflecting care and services provided.
• Ensures timely submission of all MDS to the state database and reviews validation reports.
• Leads Care Management Process to identify barriers to discharge and achieve optimal resident outcomes.
