Qualifications

• ASN from an accredited nursing program.
• Current RN Virginia state license.
• At least 2 years of nursing experience.
• LTC and MDS experience preferred.
• Ability to make decisions independently.
• Ability to communicate effectively.

Responsibilities

• Coordinate RN MDS processes to support patient care.
• Build relationships with patients and families to improve quality of life.
• Collaborate with care teams to deliver holistic, compassionate care.
• Provide leadership in ensuring top-notch care and documentation.
