Heritage Hall is seeking an experienced RN to join as an MDS Coordinator in a full-time capacity at our Leesburg, VA location. The role requires an active Virginia RN license, an ASN, and at least two years of nursing experience, with LTC and MDS familiarity preferred.

You will lead MDS coordination and patient-centered care decisions. You will build trusting relationships with patients and families, delivering holistic care to improve quality of life while collaborating with the care team to
