Heritage Hall Healthcare & Rehab Centers seeks an Registered Nurse to join our team as an RN MDS Coordinator. This full-time role focuses on coordinating MDS processes, building relationships with patients and families, and delivering holistic care to improve quality of life. Requires VA RN license and LTC/MDS experience.

The position emphasizes independent decision-making and effective communication within a compassionate, community-focused setting in Leesburg, VA.

Qualifications

• ASN from an accredited nursing program.
• Current RN Virginia state license.
• 2 years nursing experience.
• LTC and previous MDS experience preferred.
• Ability to make decisions independently.
• Ability to communicate effectively.

Responsibilities

• Build relationships with patients and families to support holistic care and quality of life.
• Lead and coordinate MDS-related activities as RN MDS Coordinator.
• Communicate care plans and changes to the team and families.
• Ensure adherence to nursing standards and regulatory requirements.
