Heritage Hall is seeking an RN MDS Coordinator to lead care planning and coordinate resident assessments with a compassionate team. You will build relationships with patients and families, ensuring holistic care and quality of life, while collaborating with the interdisciplinary team.

This full-time role follows a 7 am to 3 pm shift and includes comprehensive benefits such as medical/dental/vision, life insurance, and 401(k).

Qualifications

• ASN from an accredited nursing program.
• Current RN Virginia state license.
• 2 years of nursing experience.
• LTC and previous MDS experience is preferred.
• Ability to make decisions independently.
• Ability to communicate effectively.
