Qualifications

  • Current RN license required, with experience in Skilled Nursing/Rehabilitation facilities preferred.
  • At least six months of experience as an MDS Coordinator is necessary.

  • Monitor and document management of Medicare and Managed Care residents.

Major Responsibilities
  • Coordinate and participate in the completion of Resident Assessment Instruments (MDS, CAA's, and Care Plans) in compliance with federal and state regulations.
  • Monitor and document management of Medicare and Managed Care residents in collaboration with facility team members.
  • Implement innovative healthcare practices and continually improve systems and processes to achieve superior results.
Qualifications
  • Current, active, and unencumbered RN license in the state.
  • Experience in Skilled Nursing/Rehabilitation facilities preferred.
  • At least six (6) months of experience as an MDS Coordinator.
  • RAC-CT or RNAC certification preferred.
  • Must be qualified, compassionate, and dedicated to excellence in job performance.
