Job description

Job Opportunity: Lead MDS Coordinator, RN
Facility: Victoria Crossing Rehabilitation
Location: Brandon, Florida
Position Type: Full-Time, 8:00 AM – 4:00 PM
Salary: $42–$46 per hour, depending on experience

Responsibilities:
  • Quality Care Oversight: Ensure the delivery of high-quality care to our long-term care residents.

  • Comprehensive Assessment: Conduct thorough initial interviews and assessments for new residents.

  • Care Planning: Develop individualized care plans focused on both short-term and long-term goals.

  • Interdisciplinary Collaboration: Lead meetings with multidisciplinary teams, caregivers, and families to coordinate resident care.

  • Community Engagement: Work with specialists to optimize resident outcomes.

  • Leadership & Education: Guide nursing staff, communicate effectively at all levels, and conduct in-service education on MDS, RAPS, and Triggers.

  • Schedule Adherence: Maintain compliance with the MDS/Care Planning schedule for non-Medicare residents.

Qualifications:
  • Florida RN License: Valid and in good standing.

  • Experience: Minimum of 5 years of experience in a Skilled Nursing Facility as an MDS Coordinator.

  • Expertise: Strong working knowledge of MDS, PDPM, Medicare guidelines, and the care-planning process.
