We are currently seeking a full-time MDS RN Coordinator. As an MDS RN, you will maintain the standards of nursing care and implement policies and procedures of the facility and the nursing department. You will be responsible for managing the interdisciplinary team process, including developing care plans based on completed Minimum Data Sets (MDS) to ensure regulatory compliance and address residents' specific needs.

Responsibilities:

1. Complete assessments, MDS, and care plans for all assigned residents.
2. Monitor the completion of MDSs by other disciplines within prescribed timeframes.
3. Advise supervisors of incomplete or untimely assessments.
4. Initiate care plans and activities to achieve the best outcomes for residents.
5. Generate and distribute monthly care plan calendars.
6. Conduct care plan conferences for assigned residents.
7. Maintain and update care plans and assessments quarterly, adding or deleting issues as necessary.
8. Review incidents and accidents, and the 24-hour report daily to ensure care plans reflect current needs.
9. Facilitate and coordinate activities during patient care conferences across disciplines.
10. Ensure completion of 7-day look-back documentation within the prescribed timeframe.
11. Participate in facility management meetings as required.
12. Audit clinical record documentation for accuracy and timeliness.
13. Promote ongoing education to stay current with regulations.
14. Maintain knowledge of the MDS assessment process.

**Minimum of 1 Year of MDS Experience Required**
