Job Summary

Responsible to the Administrator, the MDS Coordinator manages Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) assessments, ensuring compliance with CMS and LTCI regulations.

Key Responsibilities
  • Participate in Interdisciplinary Care Plan meetings.
  • Develop and revise resident care plans based on RAI/MDS data.
  • Collaborate with healthcare team members to gather resident information.
  • Communicate effectively to ensure accurate and timely RAI/MDS submissions.
  • Prepare reports on Quality Measures/Indicators and contribute to quality improvement initiatives.
  • Implement educational plans for ongoing staff development.
