Job Description - Licensed Practical Nurse – MDS Coordinator (2506319)

Job Title: Licensed Practical Nurse – MDS Coordinator

Job Purpose:

The RN Registered Nurse in this role is responsible for coordinating assessments for each resident, including Resident Assessment Instrument (RAI), CAAs, and Care Plans. This encompasses all admission, quarterly, annual, significant change assessments, and Medicare assessments. Responsibilities include maintaining accurate and timely assessments, ensuring compliance with automation requirements, facilitating interdisciplinary team collaboration, maintaining rosters and census reports, and monitoring departmental reports.

Key Responsibilities:

  1. Facilitate communication between nursing center departments and patients/families to support effective assessment and care planning.
  2. Establish open communication with consultants and conduct effective interviews with patients, families, and staff for assessment purposes.
  3. Evaluate patient conditions and medical data to identify needs or amendments to assessments.
  4. Utilize nursing resources and literature to inform care plans on specific topics (e.g., bowel and bladder retraining, fall assessments).
  5. Coordinate assessment and care plan meetings, ensuring participation from relevant staff and family members.
  6. Develop and coordinate comprehensive care plans that address identified problems, interventions, goals, and responsibilities.
  7. Ensure timely completion of assessments and care plans in accordance with regulations.
  8. Educate staff on patient care plans prior to implementation.
  9. Review assessments and care plans for accuracy and completeness, signing and dating assessments to certify completion.
  10. Monitor RAI process for accuracy and educate staff on its procedures.
  11. Conduct physical assessments and make recommendations for rehabilitative and maintenance therapies.
  12. Participate in patient care conferences and may lead educational programs related to patient assessment and care.
  13. Assist in compliance with federal, state, and local regulations concerning patient care plans.
  14. Collaborate with case managers and other professionals for residents using managed care and insurance plans.
  15. Participate in quality assurance and performance improvement committees.
  16. Work with medical records and physicians to ensure accurate documentation for diagnosis coding and surgical procedures.
  17. Audit staff documentation and training for accuracy and timeliness.
  18. Participate in discharge planning, training, and resource provision.
  19. Uphold residents’ rights to fair treatment, privacy, and self-determination.

Knowledge, Skills, Abilities:

  • Participation in surveys and required reporting.
  • Continuing education and license maintenance.
  • Respect for residents’ rights and compliance with corporate policies.
  • Knowledge of Medicare and Medicaid requirements; computer literacy essential.
