Position Overview: MDS Coordinator

As an MDS Coordinator, you’ll play a pivotal role in coordinating and managing the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) process in compliance with CMS and LTCI regulations. Your efforts will directly influence care quality, resident outcomes, and facility success.

Key Responsibilities:

• Coordinate and revise interdisciplinary care plans based on RAI/MDS data
• Collaborate with nursing staff and other departments to establish accurate, personalized care goals
• Ensure timely and precise completion of RAI/MDS assessments and submissions
• Generate and analyze Quality Measures (QM) and Quality Indicators (QI) reports
• Participate actively in Quality Improvement initiatives
• Provide education and training on evolving policies and care practices
