About the Role

As an MDS Coordinator, you will lead resident assessments, ensure timely and accurate submissions, and collaborate across departments to enhance resident care.

Responsibilities

• Coordinate and oversee completion of resident Interdisciplinary Care Plans
• Partner with nursing staff and other departments for accurate and personalized care planning
• Ensure timely and compliant MDS submissions per CMS guidelines
• Analyze Quality Measures and Indicators using RAI/MDS data
• Champion best practices in assessment and documentation

Qualifications

• Active RN or LPN License in Georgia
• Previous MDS assessment experience (required)
• Background in long-term care or rehabilitation settings
• Strong organizational and communication skills
