Job summary

The Laurels of Forest Glenn seeks an experienced MDS Nurse to oversee the RAI process, conduct assessments, and coordinate care plans for residents. You will report to the MDS Coordinator and work with an interdisciplinary team to ensure compliant care planning.

RN licensure is required with knowledge of RAI and PPS, and prior MDS nurse experience is preferred. This role supports a collaborative, resident-centered care culture and ongoing professional development within a national provider.

Qualifications

• RN license required; knowledge of the Resident Assessment Instrument (RAI) and PPS is highly preferred.
• Experience as an MDS Nurse is preferred.
• Ability to coordinate PPS meetings and work with an interdisciplinary team.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling of the RAI process with the interdisciplinary team.
• Assesses residents through physical assessment, interview and chart review.
• Discusses resident care needs with physicians, nursing, social services and therapy staff.
• Reviews hospital data and outside information to inform assessments and care planning.
• Initiates significant changes to MDS when needed and supports PPS meetings.
