Job summary

The Laurels of Fulton is seeking an experienced MDS Coordinator to oversee the RAI process, conduct assessments, and coordinate care plans for residents. You will supervise the MDS Nurse and collaborate with the interdisciplinary team to ensure compliant, timely documentation.

As a key member of a dedicated care team, you will stay current with AANAC requirements and support PPS meetings to optimize Medicare days and RUG utilization. This role emphasizes communication and quality of care.

Qualifications

• Registered Nurse with active licensure.
• Knowledge of RAI process and PPS strongly preferred.
• Experience as an MDS Nurse in a skilled nursing setting.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team.
• Assesses residents through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’s.
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
