Job summary

The Laurels of Forest Glenn is seeking an experienced MDS Nurse/Care Management Nurse RN or LPN to oversee the RAI process and coordinate care plans for residents in Garner, NC. This role supports ensuring compliant assessments and interdisciplinary collaboration.

The position offers a sign-on bonus and opportunities to advance in a national provider of skilled nursing and related services. Qualifications include RN or LPN licensure and MDS/RAI knowledge.

Qualifications

• Nurse licensed to practice in the state.
• Experience with MDS processes and care planning preferred.
• Knowledge of RAI/ PPS concepts is beneficial.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team
• Assesses residents through physical assessment, interview and chart review.
• Discusses resident care needs with care givers including physician, nursing, social services, therapy, dietary and activity staff.
• Reviews information from hospital, consults and outside agencies for assessment and care planning.
• Coordinates and initiates significant changes in MDS processes.
• Maintains PPS board for monitoring Medicare days and RUGs in absence of the Care Management Coordinator.
• Remains current with AANAC requirements.
