Job summary

The Laurels of Forest Glenn is seeking an experienced MDS Nurse/Care Management Nurse RN or LPN to oversee the RAI process and coordinate comprehensive care plans for residents. The role reports to the MDS Coordinator and collaborates with an interdisciplinary team across nursing, therapy, social services, and dietary staff.

Responsibilities include completing MDS, CAA’s, PPS meetings, and maintaining the PPS board while ensuring compliance with Medicare schedules.

Qualifications

• Licensed RN in the state
• Knowledge of the Resident Assessment Instrument (RAI) process and PPS basics preferred
• Previous experience as an MDS nurse or in a similar care-management role

Responsibilities

• Completes MDS, CAA’s and care plans within required timeframes.
• Coordinates scheduling of the RAI process with the interdisciplinary team
• Assesses residents through physical exams, interviews and chart reviews
• Discusses resident care needs with physicians, nursing, social services, therapy, dietary and activity staff
• Reviews hospital and outside information to inform assessments and care planning
• Maintains PPS board and monitors Medicare days and RUGs when coordinator absent
• Keeps current with AANAC requirements

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with American Association of Nursing Assessment Coordinators (AANAC) requirements.

Qualifications

• Registered Nurse (RN)
• Knowledge of the Resident Assessment Instrument (RAI) process, including the principles the Prospective Payment Process (PPS) strongly preferred.
• Experience as an MDS Nurse
