Job summary

The Laurels of Fulton is seeking an experienced MDS Coordinator to oversee the RAI process and coordinate care plans for residents. The role supervises the Care Management Nurse and MDS Nurse, ensuring regulatory compliance and timely assessments.

As part of a collaborative care team, you will coordinate interdisciplinary input, complete MDS/CAPs, PPS meetings, and maintain PPS board while staying current with AANAC requirements. RN with MDS experience strongly preferred.

Qualifications

• Registered Nurse with MDS experience preferred.
• Knowledge of the RAI process and PPS strongly preferred.
• Experience coordinating interdisciplinary care teams.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team
• Assesses residents through physical assessment, interview and chart review.
• Discusses resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies and uses such information in the completion of the assessment and care planning.
• Coordinates, identifies, and/or initiates significant change MDS’
• Is prepared to conduct PPS meetings maintaining MDS assessments per Medicare schedule and maintains PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator
• Remains current with American Association of Nursing Assessment Coordinators (AANAC) requirements.
