The Laurels of University Park is seeking an experienced MDS Nurse to lead the RAI process and coordinate care plans for residents. This role requires strong organization, autonomy, and clinical involvement beyond bedside nursing.

Responsibilities include completing MDS/CAA and care plans, collaborating with the care team, reviewing hospital and outside information, and maintaining PPS schedules and MDS assessments. RN or LPN license required with prior MDS experience.

Qualifications

• RN or LPN license required.
• Experience with MDS nursing and RAI processes preferred.
• Familiarity with PPS and Medicare timelines.

Responsabilités

• Manages RAI/MDS process and care plan coordination.
• Performs/oversees MDS assessments within regulatory timeframes.
• Collaborates with physicians, nursing, social services and therapy staff.
