Job summary

Veracity Ventures Inc. in Petaluma, CA seeks an experienced RN Manager - Care Management to lead inpatient care coordination and discharge planning in an acute hospital setting.

The role requires strong leadership, clinical oversight, and collaboration with physicians, nurses, social workers, patients, and payers to improve transitions of care and patient outcomes while addressing social determinants of health.

Qualifications

• Bachelor’s degree in Nursing (BSN) required.
• Active California RN license required upon hire.
• Minimum of 5 years of experience in a healthcare role.
• Minimum of 3 years of case management experience in an acute care, inpatient, or hospital setting.
• Minimum of 1 year of healthcare leadership experience.

Responsibilities

• Lead and support the care management team during patient care coordination and discharge planning.
• Collaborate with physicians, nurses, social workers, patients, caregivers, payers, and community partners.
• Facilitate transitions of care and continuity across healthcare settings.
• Identify barriers to discharge including psychosocial needs, mental health concerns, substance use, and resource limitations.
• Promote patient-centered care respecting patient rights and self-determination.
• Support efficient resource utilization and care coordination throughout the patient journey.
• Provide leadership and support to care management staff.
• Ensure compliance with organizational policies, clinical standards, and applicable regulations.
