Job summary

Veracity Ventures Inc. in Petaluma, CA seeks an RN Manager - Care Management to lead care management operations and coordinate safe patient transitions across the healthcare continuum.

You will work with physicians, nurses, social workers, payers, patients, caregivers, and community resources to optimize outcomes. The ideal candidate has BSN, California RN license on hire, 5+ years in healthcare, 3+ years in acute care case management, and 1+ year in leadership.

Qualifications

• BSN required and California RN license required upon hire.
• 5+ years of healthcare experience.
• 3+ years of Case Management in an acute care hospital setting.
• 1+ year of healthcare leadership experience.

Responsibilities

• Lead and support Care Management/Case Management staff.
• Oversee care coordination, discharge planning, and transitional care.
• Collaborate with multidisciplinary teams and external partners.
• Address clinical, psychosocial, financial, and social barriers to care.
• Monitor quality, outcomes, compliance, and operational performance.
• Coach and develop staff while supporting process improvement initiatives.
• Promote patient-centered care and effective resource utilization.
