Qualifikationen

• Associate degree in nursing required; bachelor preferred.
• 2-4 years of clinical nursing experience in hospital/home health/nursing home.
• 2-4 years of care management experience preferred.

• Review medical records daily to assess admission and stay.
• Coordinate with IDT for effective care transitions.
• Develop discharge plans and post-hospital placement.
• Refer cases to physicians/managers when criteria not met.
• Act as liaison with community agencies for transitions.
• Document case management activities and compliance.
• Assist patients, families, and physicians with post-hospital options.
• Maintain attendance and comply with policies.
