Responsibilities

• Conduct initial and comprehensive resident assessments and updates to ensure care plans meet needs and preferences.
• Develop, implement, and evaluate individualized care plans with the interdisciplinary team.
• Coordinate with healthcare providers, families, and other stakeholders to ensure continuity of care.
• Provide hands‑on nursing care, including medications, treatments, and therapies, and assist with activities of daily living.
• Monitor health status and report changes to the charge nurse or physician.
• Participate in quality improvement initiatives and maintain accurate resident care records.
