Job summary

Elevance Health is seeking an LTSS Svc Coordinator- RN Clinician in Topeka, KS. This role supports member care across physical health, behavioral health, and social services, with flexible virtual work and required in-person training.

The RN will develop and monitor care plans, coordinate with non-RN clinicians, and ensure access to services within contract and benefits, while adhering to state licensure and company policies.

Qualifications

• HS diploma or equivalent with at least 3 years in care coordination or similar role; experience with chronic illnesses is preferred.
• Current, active RN license in applicable state(s).
• MA/MS in Health/Nursing is preferred where state law and contract allow.

Responsibilities

• Conduct telephonic or in-person clinical assessments to identify, evaluate, and manage member needs across physical health, behavioral health, and social services.
• Coordinate care with the member and health care team to optimize outcomes and resource use.
• Develop, monitor, and revise individual care plans to meet member needs and goals.
• Supervise non-RN clinicians (LPN/LVN, MSW, etc.) and coordinate services within the benefits structure.
