Job summary

Elevance Health in Topeka, KS is seeking an LTSS Svc Coordinator- RN Clinician to oversee member care across the continuum. This hybrid, virtual role requires clinical assessments, coordination with the health team, and development of care plans.

You will supervise non-RN clinicians, manage chronic conditions, and ensure compliant, cost-effective service delivery. Occasional travel to worksites may be required.

Qualifications

• HS diploma or equivalent with 3+ years in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role
• Active RN license in applicable state(s) required
• Willingness to travel to worksite and other locations as necessary

Responsibilities

• Perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination and management of member needs.
• Identify high-risk members and coordinate care with the health care team.
• Manage members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective use of health benefits.
• Develop short and long-term goals with the member and care team; adjust care plans as needed.
• Provide direction and oversight to non-RN clinicians in coordinating services for the member.
• Assist in authorizations/referrals within benefits structure or through contracts as permissible.
