Job summary

RYZE at Homewood in Homewood, IL is seeking a dedicated LPN/RN to join our compassionate care team for the PM shift. You will provide comprehensive nursing care to residents, supervise CNAs, administer medications, and ensure regulatory compliance in a skilled nursing setting.

The ideal candidate is a graduate of an accredited nursing program with an IL LPN or RN license in good standing, strong assessment and communication skills, and a commitment to resident‑centered care.

Qualifications

• Graduate of an accredited nursing program.
• Current Illinois LPN or RN license in good standing.
• Experience in skilled nursing, rehabilitation, or long‑term care preferred.
• CPR certification preferred or required by facility policy.
• Strong assessment, clinical judgment, and critical thinking skills.

Responsibilities

• Provide comprehensive nursing care to residents during the PM shift.
• Assess resident conditions and collaborate with the interdisciplinary team to develop, implement, and update individualized care plans.
• Administer medications, treatments, and therapies as ordered by the physician.
• Monitor residents for changes in condition and promptly communicate concerns to the appropriate healthcare provider.
• Supervise CNAs and other assigned staff, providing guidance and support throughout the shift.
• Delegate nursing assignments appropriately and ensure quality resident care is delivered.
• Accurately document resident assessments, treatments, medication administration, and significant observations in the EMR.
• Educate residents and family members regarding medications, treatments, and care plans.
• Serve as Charge Nurse by addressing clinical concerns, supporting staff, and coordinating resident care during the shift.
• Ensure compliance with infection prevention protocols, safety procedures, and all regulations.
• Collaborate with physicians, therapists, social services, dietary, and other departments to provide high‑quality resident care.
• Participate in shift reports, care conferences, and required in‑service education.
• Perform other duties as assigned.

• Ability to work effectively with geriatric and rehabilitation populations.
